Reinstalling doesn't rebuild the toolbar menu, toolbar has no "Reset Toolbar". #70

What steps will reproduce the problem?
1. Delete all items from the NiftyPerforce toolbar menu.
2. Uninsall NP
3. Reinstall NP
What is the expected output?
The toolbar should have been restored.
What do you see instead?
The toolbar was empty.
What version of the product are you using? On what operating system?
1.3.6 Vista Ultimate.
Please provide any additional information below.
Another addin had somehow corrupted the NiftyPerforce toolbar menu
(PhatStudio), showing it's menu entries instead of NP's. After uninstalling
both addins and manually deleting all related addin files, I had to manually
rebuild this menu after reinstalling NP.
Also, after reinstall, there were two of each NP command items in the
Customize->Add Command dialog.
