All log entries for the same logical request share a correlation ID across retries and failovers.

Depends on: #118 (US1 — needs request_id infrastructure)
Tests (TDD — Red Phase)
- T106-T109: Correlation ID, retry_count, fallback_chain tests
Implementation (Green Phase)
- T024-T031: Retry tracking, fallback chain building, log level assignment
Acceptance Criteria
- Retries share same request_id with incrementing retry_count
- fallback_chain shows backend progression
- WARN for retries, ERROR for exhausted retries
