NAME OF PROGRAM PURPOSE Why is the needs assessment being done? Who will be involved in the process? DISASTER PREPAREDNESS Needs assessment is being done to evaluate how far the community people know on what to do when a disaster happens. Especially nowadays many catastrophic events occur in our country. The facilitator and the learner (community people) Once the assessment is done, both the facilitator as will the community can benefit from the needs assessment. First, to identify the community on where to conduct the assessment, then the permission from the community leaders as well as the people, then the information to be disseminated should be ideal to what is happening in reality. In gathering the data a form of questionnaire will be given as well as let the people do a return demonstration. The priorities are to let the people learn the dos and donts during disaster as well on how to prepare themselves before it occurs. The ADDIE model can be use for this. The ADDIE model can be use for this. In financial matter, the facilitator should prepare an expected amount for the whole process, should also allocate enough time as well as do some research on what information will be used.
AUDIENCE Who will use the results of the needs assessment?
ISSUES What issues should be the needs assessment address organizational, accreditation, program design? What strategies shall be used to gather data? What are the priorities?
RESOURCES What resources are available to design, implement, and analyze the assessment? Do instruments exist or need to be developed? What finances, personnel, time and expertise are required?
DATA COLLECTION What types of data should be collected? Who will collect the data? What sources of information will be used? What is the timeline?
The data collection will be done before and after the implementation stage. Which means the data collected are the The facilitator who will conduct the said health teaching. The questionnaire should be created using the standards of a modified questionnaire that is already available. The timeline for this will be a month or two from planning to evaluation.
