AFFIDAVIT OF UNDERTAKING

I, NARCISO ERWIN CALIMPUSAN, of legal age, Filipino, married and a


resident of Tabalong, Dauis, Bohol, Bohol, Philippines after having been sworn to
in accordance with law, hereby depose and say:

1. That I am the Engineering Department Manager of Alturas


Group of Companies;
2. That the delivery of the sewage treatment plant equipment
will be on the second week of April 2018;
3. That two weeks after the delivery, construction and
installation of the sewage treatment plant commenced at
Colonnade-Mandaue;
4. That due to difficulty in lifting and positioning of the sewage
treatment unit, the construction and installation has not yet
started;
5. That the operation of sewage treatment plant will start on the
last week of August 2018;
6. That this Affidavit of Undertaking is executed in compliance
with the requirements of environmental certificate to be
issued by the CENRO Office, Mandaue City;
7. That I am executing this Affidavit of Undertaking to attest the
veracity of the foregoing.
