Teacher Observation Rating Sheet

This document is a teacher observation rating sheet that will be used by an observer to evaluate a teacher's performance in 9 areas related to classroom instruction and management. The observer will rate each of the 9 indicators on a scale from 1 to 7 based on how well the teacher demonstrated that skill during the classroom observation. The observer is also provided space to include any other comments. Upon completion, the observer and teacher will both sign the rating sheet.

1. Rate each item on the checklist according to how well the teacher performed during the classroom
observation. Mark the appropriate column with a (/) mark.
2. Each indicator is assessed on an individual basis, regardless of its relationship to other indicators.
3. Attach your Observation Notes Form to the completed rating sheet.
