In AGREEMENT to be bound by the following Terms and Conditions as prescribed in ANNEX A, also known as the Terms and
Agreement of Services. As such, I/we affix our signature on all pages as Final Contract, unless specified otherwise by the
Provider and the Purchaser of services.

Annex A

Terms and Conditions

1.) Services- Purchaser understands that the services indicated herein is limited to:

a.) Band performance at the church, chapel, garden or venue of the ceremony and reception shall provide ample
shade from direct sunlight or rain.
b.) Equipment and instrument tuning and placement shall be done by the provider with contingencies for sound
monitors provided by the sound contractor. Please note that certain churches or event venues vary. For instances
where churches or chapels require certain sound technicalities, the provider may cover musical instruments and
one amplifier only, so much so, the coordinator of the venue and event must coordinate such.
c.) Number and details of the performance is agreed upon prior to the event, hence, any change of song choice or
details of the performance must be coordinated by the Purchaser or their choice of Coordinator at least seven (7)
days prior to ensure that the Provider will be able to accommodate and study the pieces. Moreover, consideration
of the church prohibitions and regulations in terms of songs or hymns must be strictly followed. Furthermore, song
choices not included on the official repertoire can only be requested with the maximum of three (3) songs or hymns
as these are to be studied ahead of time.
d.) If any member/s has/ve not been chosen upon the signing of this agreement, the Provider reserves the right to do
modifications on the performance or song choice on behalf of the Purchaser. Therefore, Acts of God, sickness,
emergencies in relation to a band member/s, the provider reserves the right to make adjustments as they deem
necessary.
e.) The band reserves the right to decline services from sound providers which have poor quality equipment as such
may affect the performance, unless agreed upon and stated in this agreement.
f.) Jammers- should the couple, guest or any person who wishes to join the band as a jammer, such will be limited to 3
numbers with accompaniment; in excess thereof, the provider shall require a minimum fee of Php 300.00 per
musician with restrictions of the knowledge of the song choice and two (2) songs.
g.) Rehearsals- should the couple require a rehearsal with the band prior to the event, provider shall charge Php
500.00 per artist within Bacoor Cavite area.
h.) Recordings- audio or video recording shall be prohibited during the event itself. This is in relation to specific
copyright protection laws; therefore such recordings must be limited to personal use; moreover no copyright
infringement is intended. Please bear in mind that these events are private functions, hence the aforementioned.
i.) Agreed type of service/s´ alteration or cancellation is subject to T&C #2
2.) Payments- Provider reserves the right to decline or make advance payments or deposits as provisions to:
a.) Advance Payments/Down payments: at least 20% of the contract price should be made in cash, or check dated the
same date with the following details or bank deposits. Also, no check payments shall be accepted upon the
completion of performance
b.) Advance Payment/Down payments shall be considered as Booking Fee, hence non-refundable.
c.) Full Payment shall be required/collected by the Provider by the end of every event.
d.) Cancellation of event shall be notified at least one month prior to the event, otherwise the provider reserves the
right to charge in full. This is to maximize the booking accommodation of the band.
e.) Termination - Updates not made within a week prior to the event, the provider reserves the right to TERMINATE
the contract.
f.) Crew Meal: The Provider accepts only Cash (PhP200.00) per head, Buffet or Packed
Meal.
g.) Parking: The Purchaser must allocate two (2) parking passes in instances such as limited hotel parking spaces, or as
agreed upon.
h.) Incidentals: The Provider reserves the right to demand for payment for any equipment (owned by the band)
damaged by any guest, relative of the couple or a third party equivalent to a brand new item on its current market
value.
3.) Addendum- Any addition or requests on this contract must be made at least seven (7) days prior to the event.
4.) Familiarity- The Provider may at times be familiar with the people, crew or coordinators due to their longevity in the
business; however, this contract only covers the aforementioned details.

5.) Number and Details of Performance- Refer to the image below


By all these, both parties agree and affirm the contents, therefore affixing signatures of the
parties present on ____________, the year of _____ at__________________. Furthermore, any changes made hereafter is
subject to the Addendum Clause.
