 Birth Certificate (NSO Certified)
 Birth Certificate of Dependents (NSO Certified)
 Marriage Certificate (if Married – NSO Certified)
 BIR 1902 Form (for first time employees with no Tax ID Number)
 Tax Code Waiver
 ATM Application Form
 Group Life Insurance From / Generali Pilipinas Form
 ID Pictures (2 pcs 1x1 and 1 pc 2x2)

Secondary Requirements

The following information are to be submitted 30 days after hire date:


 Diploma or TOR or School Certification
 COEs or Clearance from previous employer (if w/ previous employer)
 Police Clearance (Original Form)

 PTEP Acknowledgement Form


 Other Acknowledgement Forms as required by Program/Client
