 Whole Body picture (size 3R)


 Accomplished Application Form/Resume with 2x2 ID picture
 Birth Certificate (PSA) with OR
 Marriage Contract of Parents (PSA Copy) with OR
 High School Diploma with dry seal (Original Copy)
 Transcript of Records with dry seal (Original Copy)
 Vocational Course/College Diploma with dry seal (Original Copy)
 AFSAT result with dry seal (if possible attach code number)
 Medical/Physical Exam Result (to be secured at CLSH)
 PFT result (to be secured at H53EBde, PA)
 Drug Test Result
 Hepa Test Result
 Clearances: shall be from the place of residence for at least five (5) years.
Barangay Clearance
Police Clearance
Mayor’s Permit
NBI
RTC
 Certificate of Legal Beneficiaries (notarized)
 Affidavit of Being Single and without child (notarized)
 BIR Tax Identification Number (TIN)
 PhilHealth Membership/Identification Card
 CENOMAR (PSA Certified) with OR, must be current and within 3 months before validation
period.
