Carrier Partner FMiP Unlock Form

This form is used by carrier partners to request disabling the Find My iPhone lock on a device when a customer has provided valid proof of purchase. The carrier partner must complete the form with the customer's product serial number, IMEI, description, date and location of original purchase, receipt number, retailer information, and attach the proof of purchase. All fields are required and the completed form should be submitted to the appropriate regional email address listed. The customer's warranty rights are not affected by validating the proof of purchase.

 Disable Find My iPhone Lock Request

This form should be completed when a repair is blocked in GSX due to FMiP device lock and where the customer
has provided the valid proof of purchase proving device ownership. The Carrier Partner should complete this form
and attach the proof of purchase receipt of the product’s original retail purchase.
