Will the lift plan require calculations/input from a 3rd party technical expert?
Will the lift involve a marine vessel?
Could the lift be affected by a proximity hazard (e.g. resitricted area, power lines, excavation,
underground service, public road, etc)
Will the crane utilization be above 80%?
Will load pass over, or come close (3m) to live plant (e.g. generator), when crane utilization is above
70%?
H Is fixed lifting appliance (e.g. runway beam, gantry) utilization above 90%?
I Will there be less than 1m boom clearance from obstructions that are non-electrically charged?
G Will ground/deck bearing pressure be more than 80% of the ground/deck bearing capacity?
H Will personnel be lifted or lowered?
Is the lift to be peformed by a lifting crew not familiar with the facility?
Will the lift take more than one shift to complete?
Is Pedestal Crane whip line utilization over 90% or main block over 80%?
Is lift affected by proximity to hazards (road, overhead power lines, etc)
Is the lift determined as high risk by the site?
Will the lift involve tandem / multi-crane lifting?
If you answered "Yes" to any question, lift as determined by the site as High Risk.
Are lift characteristics outside of normal operations?
Will 3rd party contractors (Mode 3) carry out the lift?
Will the lift involve vehicle drivers?
Will the lift involve more than one banksman?
Does the load require slinging? (i.e. no certified lifting point) If so, does the load have any hazards?
(e.g sharp edges)
Is the weight of the load estimated?
Will the lift plan require simple calculations?
Will environmental conditions place restrictions on the lifting operation?
M During the lift, will the crane operator's view of the load be restricted?
E Will load pass over, or come close (3m) to live plant (e.g. generator), when crane utilization is below
D 70%?
I Will the lift involve cross hauling?
U Weight distribution is uneven, or can shift?
M Will the lift involve topping & tailing / overturning load with two or more appliances?
Does the load have a high CoG, offset CoG, or lifting points below CoG?
Is the load fragile, requires temporary supports, or load integrity uncertain?
Does the load have an offset CoG without special slings to compensate or is it an akward shape or
have a large sail area?
Are slings to be used at an angle below 60 deg. from the horizontal?
Is the lift NOT identified as high or low risk?
Is the lift determined as medium risk by the site?
Does the lift involve a load longer than 40ft?
If you answered "Yes" to any question, lift as determined by the site as Medium Risk.
None of the above apply
Lift personnel have the requisite training / competence. Lifting equipment and the load are simple to

Processes and procedures are in place / functional and loss in the event of an incident or accident is

If you answered "Yes" to all questions, lift as determined by the site as Low Risk.
