1.0 LIFTING OPERATIONS MANAGEMENT STANDARD
1.1 Introduction
This standard sets out the minimum mandatory requirements relating to the control of lifting operations being
carried out at locations that are either owned or operated by the Rumaila Operating Organisation in Iraq.
The standard sets out high level requirements for the implementation of a safe system of work in relation to
lifting operations, and is intended to be reviewed by ROO DLT Management Team.
1.2 Purpose
The purpose of this document is to set out minimum mandatory rules for the control of lifting operations and
equipment.
The document contains the rules outlining the way in which lifting operations and equipment will be managed
and controlled and is not designed to offer specific detail on how lifting operations are to be performed.
1.3 Application
This standard is applicable to all activities involving the use of lifting equipment and the performance of lifting
operations being carried out within ROO owned or managed facilities and are controlled through the
“Worksafe” system
It applies to all lifting operations and equipment including new, existing, second-hand and leased equipment
and applies to all ROO Departments i.e. Projects, Operations and Wells

Note - 3 party locations, for example, areas that have been ring-fenced for drilling / workover activities, may
use their own standards and procedures for lifting operations and equipment, provided they follow
the intent of this standard.
1.4 Definitions
Appropriately Indicates the level of management and oversight by a Competent Person that
Supervised personnel executing a lifting operation require in relation to their level of training and
competence and the complexity of the lifting operation
Approval Signifies confirmation by a Competent Person that a lifting plan is safe to execute,
mitigates all risks and hazards, provides a safe system of work and is in accordance
with regulatory and company requirements
Authorisation Signifies confirmation that the lift has been correctly categorized, that the lifting plan
has been developed correctly, that the correct management processes have been
followed during the stages and has adequately considered and mitigated all
personnel safety and process safety risks
Authorised to Process of examination and approval that is carried out on all mobile lifting
Operate (ATO) appliances used within ROO owned facilities signified by a “ROO Approved” sticker
Cargo Carrying Units Portable equipment with a dedicated sling set for repeated use in the transportation
of goods to and from ROO facilities
Certified Signifies that the equipment has valid certification
Certification Is approved and legally compliant documentation providing written evidence that an
item of equipment meets a required standard or process
Colour Code A method of marking equipment, normally with paint or coloured tag, to give a visual
indication of its certification status

Competent Person An individual who, by way of education, training, experience and defined
assessment, is knowledgeable of applicable standards, is capable of identifying
workplace hazards relating to the specific operation, is designated by the employer
and has authority to take appropriate actions
Contingency Plan Is a documented process that defines the potential worst case consequences in the
event of a dropped object or catastrophic failure of the lifting equipment,
documenting the mitigation measures to be taken prior to the lift and the emergency
response actions required in such an event
Direct Supervision Signifies constant management and oversight by the Competent Person in the
immediate proximity of lifting operation
Endorsement Signifies confirmation that a lifting plan is safe to execute, it mitigates all risks and
hazards, provides a safe system of work, is in accordance with regulatory and
company requirements and that all required technical support has been provided
and incorporated
Factor of Safety Is the relationship between the Safe Working Load (SWL) or Working Load Limit
(WLL) and the Minimum Breaking Load (MBL)
Lifting Accessory A device that is used or designed to be used to connect a load to a lifting appliance
and which does not form part of the load e.g. wire sling, fibre sling, shackle, hook
etc.
Lifting Appliance A device capable of lifting or lowering a load and includes its attachments used for
anchoring, fixing or supporting it.
Mobile Appliance Equipment of this type is self-propelled and will be able to
travel from site to site and or move around the site under its
own power or be pulled into position by another vehicle e.g.
forklift trucks, side boom, mobile crane, and mobile work
platform
Portable Appliance Equipment that can be transported from site to site and
used on a number of different loads. Equipment falling into
this category can generally be handled by an individual
such as chain hoists, lever hoists etc.
Lifting Operation An activity that uses lifting equipment to raise, lower or suspend a load
Lifting Personnel Refers to any operation where a lifting operation takes place using a person as a
load. Typically involving the use of a mobile crane via a basket or a MEWP (Mobile
Elevating Work Platform)
Load Means any material, people or animals, or any combination of these, that are lifted
by lifting equipment
Major Lift Means a lift that requires independent assurance and is considered more complex or
larger than is normally performed
Management of Is a documented process that provides a route for personnel to follow, which is

Mobile Lifting For the purposes of this procedure, this term means any powered mobile mechanical

Pulling and Means an operation where, in the event of failure of the lifting equipment, the load

Rescue Plan Is a documented process that details the method of retrieving personnel in the event
of an accident or incident
Rigging Operation Means the use of portable lifting appliances and accessories to lift, lower, cross-haul
and rotate loads
Shall Is a mandatory term where no deviation is permitted without written approval from
the Lifting Technical Authority or Authorised Person
Should Denotes as an advisory term or recommendation, where consideration could be
given to a suitable alternative
Proof Load Test Is a load that is applied to the lifting equipment in excess of its rated capacity in
order highlight any defects and to prove that the equipment is fit for further use. This
can only be carried out by a Competent Lifting Equipment Inspector
Thorough Signifies a visual and function check without load and, where appropriate, testing, for
Examination the purposes of certifying that the equipment is safe to use. Note – The person

carrying out this type of examination may be employed from a specific 3 party
Inspection Company, or by the equipment owner, provided that he/she can
adequately demonstrate their competence in performing such work and act with the
necessary degree of impartiality and independence
Uncertified Refers to an item used for lifting, lowering or supporting a load that does not have
valid certification or supporting calculations
Written Examination Is a suitable written examination process, drawn up by a Competent Person, that
Scheme determines the frequency and type of examination carried out on an item of lifting
equipment by a qualified Inspector, that is dependent on certain circumstances e.g.
maintenance, defects, usage etc.

2.0 GENERAL
This document sets out mandatory requirements associated with all lifting operations taking place and
equipment that is in use at locations and facilities owned or operated by the Rumaila Operating Organisation
within Iraq.
In addition, this document also directly relates to the ROO Procedure for Lifting Operations ROO-ALL-HS-
PRO-0052.
This standard is split into a number of different categories aimed at Management, Supervision and User
levels.
Each of these categories is split into three different areas that relate to people, equipment and process;
People Includes requirements for personnel training, competence and selection processes. It is
designed to ensure that all personnel carrying out this type of work related activities have
received the correct level of training.
Equipment Designed to ensure that all equipment used within ROO is of adequate quality and is
designed and manufactured to the correct standard.
Process This section contains information on the correct selection and use of lifting equipment and
the minimum acceptable design standard

3.0 LIFTING STANDARD REQUIREMENTS
3.1 Personnel
3.1.1 All personnel supporting and performing lifting operations shall:
o Have clearly defined roles and responsibilities
o Have achieved a defined level of training in order to perform their roles and
responsibilities
o Understand site specific lifting and control of work processes
o Be periodically assessed to verify that training levels are maintained
3.1.2 Where an individual has not yet achieved the defined level of training, they shall only assist in
supporting and executing lifting operations while under the direct supervision of a trained and / or
competent person.
3.1.3 Deviation from minimum approved roles, responsibilities, training and competence levels shall be
endorsed by the ROO Lifting Technical Authority or Authorised Person.

3.2 Plant and Equipment
3.2.1 All lifting equipment shall be:
o Designed, manufactured or adapted in accordance with ROO procedures, international
standards and be suitable for the purpose for which it is to be used
o Of adequate strength and stability with a suitable factor of safety for each load. In
addition, every part of a load and anything attached to it and used in lifting shall also be of
adequate strength and have a suitable factor of safety
o Provided, where appropriate, with equipment or devices such as rated capacity indicators
and / or rated capacity limiters, when there is a significant risk from overturning and / or
overloading arising from the use of the equipment. Such devices shall provide audible
and / or visual warning when the safe lifting limits are being approached
o Positioned or installed to reduce to as low as reasonably practicable, the risk of the
equipment or a load striking a person, falling freely or being released unintentionally
o Positioned, set up or installed taking into consideration the strength of the ground or the
surface on which the lifting equipment is positioned or located and the stability of the
equipment under all load conditions
o Maintained and, where required, periodically dismantled, thoroughly examined, inspected
and tested in accordance with ROO procedures, manufacturer’s guidelines and the
requirements of a Competent Person
3.2.2 All lifting equipment shall be subjected to a pre use inspection by the user of the equipment before
and after each use
3.2.3 Lifting equipment for lifting persons shall be designed, manufactured or adapted:
o To prevent a person using or carrying out an activity from it, from being crushed, trapped,
struck by an object or falling from the carrier
o To have suitable devices to prevent the risk of a carrier falling
o So that any person trapped in the carrier is not exposed to danger and can be freed
o With an enhanced factor of safety or safety factor of twice that used for general lifting
purposes

3.2.4 Lifting equipment shall be clearly marked:
o To indicate its Safe Working Load or Working Load Limit.
o With the relevant information relating to its SWL or WLL dependant on its configuration or
provided with information which shall be kept with the equipment at all times
o With a unique identification mark to allow traceability to certification
o With a colour coded to indicate certification status
o With any other markings appropriate for reasons of health and safety
o To clearly indicate that it is suitable for the purposes of lifting personnel, if the equipment
is designed for this purpose
3.2.5 All mobile lifting appliances shall, where applicable, have an current genuine ATO (Authorised to
Operate) approval
3.2.6 All lifting equipment shall be subjected to a thorough examination by an Independent Competent
Person periodically within its lifetime
o Lifting Accessories 6 monthly
o Portable Lifting Appliances 12 monthly
o Mobile Lifting Appliances 6 monthly
o Be thoroughly examined, If dependent on the installation conditions, after installation and
before being put into service for the first time, after assembly and before being put into
service at a new site or in a new location, to ensure that it has been installed correctly
and is safe to operate
3.2.7 Any lifting appliance used for the lifting or transport of personnel, shall be examined on a 6 monthly
schedule
3.2.8 Uncertified steelwork and lifting points shall only be used after an assessment, approved by a
Structural Engineer or ROO Lifting Technical Authority or Delegated Person. If the steelwork is
used on a regular basis to perform a lifting operation, it shall be certified
3.2.9 Lifting equipment certification shall be securely retained for as long as the company operates the
equipment. Certification should be retained for a minimum of two years after the last date of use

3.2.10 Certification shall be made available, upon request, to authorized personnel.
3.2.11 Certification shall contain sufficient information to identify:
o The name and address of the company who carried out the thorough examination and /
or test
o The location of the equipment
o The name and qualifications of the Competent Person (author) making the report along
with the name of a person signing or authenticating the report on behalf of its author
o The date of the report
o Full description of the lifting equipment, which has undergone the thorough examination
and / or test
o The date of the last thorough examination and / or test
o The Safe Working Load for any configurations of Safe Working Loads and the last
configuration in which it was thoroughly examined and / or tested
o All test loads and configurations
o If applicable, that the lifting equipment after thorough examination and /or test has been
installed correctly and is safe to operate
o If applicable, any part found to have a defect, a description of the defect and the details of
any repair, renewal or alteration required to remedy a defect
o That the lifting equipment has been thoroughly examined and / or tested in accordance
with a written examination scheme
o The interval of thorough examination and /or test required after the occurrence of
exceptional circumstances
o The latest date by which the next thorough examination and / or test must be carried out
3.2.12 A Competent Person making a thorough examination shall notify the ROO Lifting Technical
Authority or Lifting Coordinator in writing, as soon as is practicable, of any defect in the lifting
equipment which, in their opinion, is or could become a danger to persons
3.2.13 Lifting equipment shall be suitably stored and controlled to prevent unauthorised use, and to
eliminate or reduce the risks to the health and safety of personnel and damage to the equipment
and the environment.
3.2.14 Defective lifting equipment shall be suitably controlled and not used before the defect is rectified

3.3 Process
3.3.1 Lifting operations shall be:
o Planned by Competent Personnel
o Risk assessed (HITRA)
o Carried out under a Permit to Work / Standard Operating Procedure (SOP)
o Categorised
o Appropriately supervised
o Carried out in a safe manner
3.3.2 Lifting plans shall contain sufficient information to demonstrate operational control and that lifting
equipment can be used within its designed safe use parameters
3.3.3 Risk assessment shall identify, analyse and record the steps involved in performing a specific task,
the existing or potential health, safety and environmental hazards and risks associated with each
step, and the recommended actions, procedures and personnel roles and responsibilities that
mitigate or reduce the identified hazards and risks
3.3.4 Lifting operations shall be categorised as either routine or non-routine, in order to indicate the
required level of risk assessment, lift planning, training and competence of personnel supporting
and / or executing the lifting operation and the level of approval and authorisation
3.3.5 Lifting plans shall be reviewed and approved according to their category
3.3.6 During the planning of major lifts, a review of the lifting operation and associated documentation
shall be completed in consultation with Lifting Technical Authority or Authorised Person to design
out operational and process safety risks
3.3.7 When selecting lifting equipment, consideration shall be given to the working conditions, the risks
to the health and safety of personnel, which exist in the location that the lifting equipment is to be
used and any additional risk posed by the use of that equipment
3.3.8 Any operation involving the lifting of personnel, should be avoided if at all possible, however should
a lift of this nature be required, it shall always be considered a non-routine lifting activity and be
planned accordingly

3.3.9 Lifting personnel shall only be carried out when the risk of personnel falling from a carrier, or being
crushed, trapped or struck while being transported in a carrier has been mitigated to as low as
reasonably practicable.
3.3.10 Any operation involving the lifting of personnel, shall require the development of a rescue plan
which shall be effectively communicated to all personnel involved, along with the rescue equipment
being readily available and operational at the lift location before the lifting operation starts
3.3.11 Lifting over or in close proximity to live plant shall be avoided if at all possible, however should a lift
of this nature be required, it shall always be considered a non-routine lifting activity and planned
accordingly
3.3.12 Lifting over or in close proximity to live plant shall not be undertaken unless an operational
contingency plan is in place, risks are reduced to as low as reasonably practicable and no other
reasonable alternative exists.
3.3.13 A method(s) of communication shall be agreed prior to the start of a lifting operation and delivered
in a clear and concise manner during the lifting operation
3.3.14 Prior to starting any lifting operation, an appropriate pre-lift safety briefing (toolbox talk) shall be
carried out
3.3.15 Once a load is connected to a lifting appliance, personnel should not touch the load with any part of
their body as the load is being lifted or before the load is properly landed and any stored energy
has been released. The exception is where it has been previously approved as a result of
performing a ‘specific ’ risk assessment
3.3.16 During a lifting operation the load shall be under control at all times and personnel located in a safe
position
3.3.17 If a lifting operation deviates from the authorised lift plan and / or if additional hazards are identified
the lifting operation shall be stopped and made safe immediately and a management of change
process followed
3.3.18 Learning points recorded on lifting plans shall be reviewed and where appropriate, actions taken
and communicated to all relevant personnel and the lift plan revised
3.3.19 The Lifting Technical Authority or Authorised Lifting Coordinator shall monitor site compliance with
applicable ROO requirements, this Policy and site procedures through an system site audits /
reviews
