INTENDED LEARNING OUTCOMES
• By the end of this session, you should be able to
• Differentiate between an incident, accident, near miss and dangerous
occurrence
• Identify reasons and benefits of carrying out an incident/accident investigation
at work
• Describe the procedures of incident investigation
• Carry out an incident investigation (role play)
Theory of accidents-causes

The Domino theory

• Lack of management control – e.g. no structured training for slingers


• Individual and job factors – person slinging does not know safe technique
• Unsafe acts and / or conditions – load exceeding SWL of sling is lifted
• Accident – load falls and hits worker below
• Loss – e.g. fractured leg or even fatality
• statistically it is proven that a workplace
fatality has many previous events before a
worker is killed.
• If these are not recorded they cannot be
investigated and rectified.
• If they are reported then solutions can be
implemented at an early stage and therefore
prevent the escalation to fatal accidents
Reporting and Recording an accident

• Reasons why records should be kept:


• Monitoring trends
• Prevent a recurrence
• Legal duty to report
• Link to health surveillance
• Defend civil claim / criminal actions
Why we should Investigate
• Collate accident and ill-health data and establish trends
• Identify weaknesses in management systems
• Comply with legal requirements
• Prevent business losses
• Demonstrate concern
• Defend:
• Claims for compensation
• Criminal prosecution
Prevent future accidents
The Investigation process
Establish the facts:
• What activities were being carried out at the time?
• Was there anything unusual?
• Were there adequate SSoW and were they followed?
• Was the risk known? If so why wasn’t it controlled?
• Did the organisation and arrangement of the work influence the accident?
• Was maintenance and cleaning sufficient?
• Was the safety equipment sufficient?
