I am fully aware that University of the East (UE) or its designated representative is duty bound and obligated under the Data Privacy
Act of 2012 to protect all my personal and sensitive information that it collects, processes, and retains upon my enrolment and during my
stay in the University.

Student personal information includes any information about my identity, academics, medical conditions, or any documents containing my
identity. This includes but not limited to my name, address, names of my parents or guardians, date of birth, grades, attendance,
disciplinary records, and other information necessary for basic administration and instruction.

I understand that my personal information cannot be disclosed without my consent. I understand that the information that was
collected and processed relates to my enrolment and to be used by UE to pursue its legitimate interests as an educational institution.
Likewise, I am fully aware that UE may share such information to affiliated or partner organizations as part of its contractual obligations, or
with government agencies pursuant to law or legal processes. In this regard, I hereby allow UE to collect, process, use and share my
personal data in the pursuit of its legitimate interests as an educational institution.

• In addition, I am likewise giving my consent/permission in favor of my parents/guardian/representative or whoever is responsible in

provid-ing care for me to access, verify, examine and or inspect my academic and scholastic records, school fees/accounts in the
University, the result of my physical medical examination (PME) and all matters that relate to my status as a student of the University.

• Finally, should I commit any misconduct or should there be a complaint filed against me, before the Student Affairs Office (SAO) or
Student Disciplinary Board (SDB ) by reason of violation of the provisions of the Student Manual or any laws or ordinances, I hereby
authorize and give my full consent in favor of the University to inform my parents, guardian, representative or whoever person is in charge
of providing care or custody for me.

• Does not apply to students from Graduate School and Law School.

As a parent/guardian of UE student, I am fully aware that the University of the East (UE) or its designated representative is duty bound

D E C L A R A T I O N
1. In consideration of my/my child's admission to the University of the East, I hereby agree to recognize, accept, abide by and comply
with all its rules and regulations, policies and requirements set forth in the Code, handbooks, bulletins, prospectuses and other
official documents of the University, particularly those pertinent to the use of facilities, payment of fees, conduct and discipline.
2. I am fully aware that it is the absolute prerogative of the University to accept me/my child or not; and that willful and deliberate
disregard of elementary rules of good behaviour, decency and morality, or failure to disclose relevant facts, or deliberate intent to
give misleading information in the application for admissions, constitutes sufficient cause for denial of admission or discharge from
University.
3. I am fully aware that my/my child's enrollment is on a semester (for tertiary level) / annual (for basic education level) basis only.
4. I was duly apprised of the tuition and other fees that will be paid during the semester (for tertiary level) / year (for basic education
level) which I found to be fair and reasonable, and I agree that the University has the right to raise its tuition and other fees in
accordance with the guidelines set by the Commission on Higher Education.
5. I am fully aware that installment payments not paid on the due date shall be subject to additional surcharge based on the University
guidelines.
6. Finally, I pledge loyalty to the University and adherence to the laws of the land.

I am fully aware that installment payments not paid on due date shall be subject to additional surcharge per University policy. I
acknowledge that I/my child will be officially enrolled only after the required amount upon registration has been paid. I have read and
understood the DECLARATION and the DATA PRIVACY CONSENT FORM of this registration form. I hereby agree that if payment is made
after the due date, I/my child shall have to undergo the ENROLLMENT process again. Payments made after the date will be considered as
advanced deposit for tuition payment.
