Syllabus Topic:
Industrial Safety Analysis:
Locating injury sources, identify causes of injury, injury investigation, safety analysis techniques -
FAULT TREE, THERP, FMEA; Risk tolerability, ten-point scale for workplace hazards.
Instructions:
1. Write your answers in your own words.
2. Write answers with reference to the documents you have studied. As far as possible, references
should be mentioned as per IEEE convention for reference. (see [Link] )
3. Do not copy directly from others/ online resources.
4. Wherever possible, give diagrams/schematics, charts, tables etc. in your answer.
5. As far as possible write just required answer. Avoid unnecessary long descriptions.
Questions:
Answer following questions considering the instructions given above.
Q 1. What are the major sources of injury at occupational place?
Q 2. Explain various sources of injury at the occupational place.
Q 3. State various methods of safety analysis.
Q 4. Discuss FTA (Fault Tree Analysis) with a case study.
Q 5. Discuss ‘Technique for Human Error Rate Prediction’ (THERP).
Q 6. Discuss FMEA (Failure Mode Effects Analysis) technique.
Q 7. What are the two factors associated with risk? Explain how these two factors impact on the
selection of accident controls.
Q 8. Is it important to conduct accident investigations? Why?
Q 9. What are the six key questions that should be asked during accident investigations?
Q 10. Explain the importance of these six questions to the health and safety professional.
