Pursuant to Sec. 69.1 (i) Rule XXIII of the 2016 Revised lmplementing Rules and Regulations (IRR)
of Republic Act (R.A.) 9184, and in accordance with Department Order No. 28 series of 2024, this

Based on the Approved Consolidated Three (3)-Strike Report, dated October 16, 2024, and posted
on the DPWH Intranet on October 30, 2024, this contractor/supplier, bearing the PhilGEPS
Registration No. 201704169494032150178 and located at 123 Matimtiman Street Sikatuna Village,
C)uezon Citv, Metro Manila, National Capital Reaion (NCR), has incurred four (4) strikes/offenses
within a year from the approval date of the Resolution Recommending the Imposition of Strike One
- First Warning. While the specific sub-offense categories vary between Implementing Offices
(District Engineering Offices) - namely (i) participation in bidding with incompatible business type as
indicated in the Business/Mayor's Permit, and (ii) submission of incomplete, insufficient, or defective
bid documents with missing mandatory provisions -all four (4) violations fall under the main strike
category of "Submission of Patently Insufficient Bid."

in any DPWH bidding processes, effective immediately upon receipt of the Notice of Suspension.
This suspension shall remain in effect during any period of reconsideration or appeal until a final
decision is rendered by the Office of the Secretary. However, this suspension does not apply to the
contractor's participation in procurement activities of other government agencies.
This Order shall take effect immediately.
