Position, Manager & Start Date
Your title will be Hygiene Assistant a non-exempt position, and you will report to Araceli
Contreras We are excited to welcome you for your first working day on December 30, 2024
at 8:30am at Winchester Dental Care.
Upon beginning your employment journey with Heartland Dental, you’ll participate in our
orientation program, myCompass. myCompass offers a combination of live virtual
training, in addition to self-paced sessions. The program will be held the first two days of
your employment. This valuable session provides insight into our culture, and important
information to help you navigate around our organization. You will receive a separate
email from your manager,Araceli Contreras
, with more details.
Salary, Payroll & Annual Reviews
Your compensation will be paid in accordance with the Company’s standard payroll
schedule (which is currently bi-weekly) in the amount of $19.00/Hourly , subject to your tax
withholding information on file. Your pay can be deposited directly into your savings or

checking account, and it is deposited in accordance with the standard payroll
schedule. Although overtime is unusual, we do, of course, pay time and one-half for
overtime for hours worked over 40 hours per week, or as required by the state in which you
are employed.
Your performance will be evaluated continuously throughout your employment and you will
receive performance and compensation reviews.
Work Location & Hours
Your primary work location is Winchester Dental Care. Your scheduled hours will be as
follows:
Tuesday, Wednesday and Friday 8:30am-5:30pm. Scheduled hours are subject to change,
based on the needs of the Company.

Dental Benefits
General services in your Heartland Dental supported office are paid at 100% minus any lab
fees for employees and immediate family (spouse & dependent children up to 26 years of
age). A 15% discount is offered to extended family members (non-dependent children,
siblings & grandparents). All dental benefits are provided by your Heartland Dental
supported office.
Paid Time Off
Regular full-time non-exempt team members are eligible to earn Paid Time Off (PTO) after
one continuous year of employment. PTO is calculated in relation to the number of hours
worked in your previous anniversary year. You can schedule unpaid time off with prior
approval from your manager. At your one-year anniversary date, you will earn two weeks of
PTO that is calculated based on the hours worked the previous year. Your manager must
approve all PTO requests. The Company complies with all state and local paid leave laws.
Note for team members employed in Arizona, California, Colorado, Connecticut, Illinois,
Maine, Maryland, Massachusetts, Michigan, Minnesota, Nevada, New Jersey, New Mexico,
New York, Oregon, PA-Allegheny County (not state of PA), Washington, Washington
D.C: you will be eligible to accrue PTO starting on their hire date, subject to and in
accordance with the terms set forth in the Company’s applicable PTO policy. In your first
year of employment, you will accrue PTO at the minimum rate as defined by state or local
law. You will accrue PTO hours daily based on the hours worked the previous day. With
manager approval, PTO hours can be used after your applicable pending period. You can
schedule unpaid time off with prior approval from your manager. Full-time team members,
at your one-year anniversary date of employment, you will accrue PTO at a 2-week rate
based on your hours worked. Your manager must approve all PTO requests. The Company
complies with all state and local paid leave laws.
401(k) & Roth 401(k)
After 60 days of employment, you may participate in the 401(k) (pre-tax) and/or Roth
401(k) (post-tax) plans.
Confidentiality
You agree that you will not engage in the unauthorized disclosure of “business secrets” or
other confidential information, and that the misuse or unauthorized disclosure of
confidential and/or proprietary information not otherwise available to persons or firms
outside Heartland Dental and its supported entities is cause for disciplinary action, up to
and including termination. The protection of confidential and/or proprietary business
information and trade secrets is vital to the interests and success of Heartland Dental and
its supported offices.

Employment Relationship
Employment with the Company is for no specific period of time. Your employment with the
Company will be “at will,” meaning that either you or the Company may terminate your
employment at any time and for any reason, with or without cause. Any contrary
representations that may have been made to you are superseded by this letter agreement.
This is the full and complete agreement between you and the Company on this term.
Although your job duties, title, compensation and benefits, as well as the Company’s
personnel policies and procedures, may change from time to time, the “at will” nature of
your employment may only be changed in an express written agreement signed by you and
a duly authorized officer of the Company (other than you.)
Offer Contingencies
This offer of employment is contingent upon the successful completion of a background
check, TB test (Tuberculosis) and proof of eligibility to work in the United States as
required to complete the I-9 within three days of employment (the federally required
timeline), and if required, confirmation of employment authorization via E-Verify. Further,
this offer of employment is contingent upon your continued compliance with Company
policies in addition to local, state and federal regulations which may be in effect now or in
the future.
