We, the Grade 12 Aquino students, are conducting a research study titled, “Level of Awareness and Practices on Waste
Management of CatNHS: A Basis in School-Based Policy Implementation.” This study aims to investigate the level of
awareness and practices regarding waste management among Cataggaman National High School students. The findings will
serve as a basis for formulating and implementing effective school-based waste management policies.

In this connection, we respectfully seek your assistance and consideration in answering our survey questionnaire, your
answers will help us understand students’ level of awareness and practices regarding waste management.
