Early Intervention Process Guide

This document outlines the early intervention process from referral to transition out of services. It begins with the service coordinator contacting the family and explaining the process. An evaluation of the child and family is conducted to determine eligibility. An Individualized Family Service Plan (IFSP) is then developed with family goals and assigned service providers. FLOW CHART
REFERRAL FOR EARLY
INTERVENTION
•Service Coordinator gets assigned to family and contacts the family via preferred
communication method.
•Service coordinator explains the EI process, parents rights, eligibility, receives
consent to complete an eligibility evaluation with the family, and answers any
questions that the family may have about the early intervention process/services.
EVALUATION- FAMILY LEVEL
(RBI)
•Service coordinator (in Geauga County) completes a conversation like interview
with the family about their daily routines. The interview includes asking the family
to rank each routine from wanting to change a lot about the routine to highly
satisfied where they don’t have much or anything at all that they would change about
that specific routine.
•Family and service coordinator work together to determine which routine and skills
the family would like to prioritize.
CHILD ELIGIBILITY
EVALUATION
•Service coordinator and two professionals from different disciplines complete an
initial developmental evaluation of the child with the family.
•The team can utilize interviewing the family for some of the testing options, asking
the family to record their child doing specific tasks throughout the day. Professionals
can also coach families in administrating some of the evaluation tasks (especially
during tele-practice).
•Professionals can/should ask the family if there is anything else they should know
about their child , how they feel the evaluation went (typical or non-typical
behavior/use of skills from the child)
INITIAL IFSP
DEVELOPMENT/PLAN
•Service coordinator invite the family to the team meeting, in which the eligibility of
the child is discussed.
•The family works with the EI team to determine the best fit for primary service
provider and to determine a goal(s). The team can then determine frequency for the
primary service provider to contact the family- # of sessions in the
home/community/tele, etc.
•IFSP meeting with family, service coordinator, PSP, and if applicable SSP(s)- discuss
the IFSP document, goal(s), services and frequency, plan for initial session with PSP,
etc.
IMPLEMENTATION OF IFSP-
RECEIVING SERVICES
•Service providers (PSP) coaches the family in implementing evidence based
interventions to make progress towards the family’s IFSP goal(s).
•PSP collaborates with the family to determine their learning process, what kind of
supports they need and how much, update of family’s routines, etc.
•Service coordinators keep in contact with the family to ensure that they are receiving
the right supports and if they need/would like additional resources/supports.
•Data collection- PSP can coach family in collecting data regarding target behavior
REVIEW OF
IFSP/GOALS/PROGRESS
•The EI team invites the family back to team meeting to discuss updates-progress,
setbacks, needed changes, etc.
•Family collaborates with the EI team to determine if the previous IFSP goal has been
met, and if so what they would like their new goal(s) to be. If the goal has not been
met the family and EI team can reflect on why that might be-discuss possible needed
changes to make progress towards the goal.
REPEAT OF
IMPLEMENTATION- SERVICES
•Service providers (PSP) coaches the family in implementing evidence based
interventions to make progress towards the family’s IFSP goal(s).
•PSP collaborates with the family to determine their learning process, what kind of
supports they need and how much, update of family’s routines, etc.
•Service coordinators keep in contact with the family to ensure that they are receiving
the right supports and if they need/would like additional resources/supports.
•Data collection- PSP can coach family in collecting data regarding target behavior
TRANSITION TO PART B- PRESCHOOL
SERVICES/EXIT PART C EARLY
INTERVENTION SERVICES
•PSP and service coordinator prepare family for the transition to preschool services or
out of services completely. Service coordinator has a discussion(s) with the family
about their options once their child turns age 3 and ages out of part C early
intervention services. Conversation about what the next steps might look like.
•Preschool eligibility- EI team (service coordinator and PSP/SSPs) can join the
family for the preschool eligibility evaluation and/or IEP meeting.
