I, JASON ABARIENTO CABIAS, of legal age, single, and a resident of Real


Street, Tacloban City, after being duly sworn according to law, do hereby depose
and state that:
1. I am a the owner of a PASSPORT with number EB3447907;
2. On November 8, 2014, I lost my PASSPORT while I was in my business trip
at General Santos City;
3. Despite diligent search and best efforts, I failed to locate and recover my
PASSPORT to this day;
4. Said PASSPORT was by no means confiscated for any reason;
5. Wherefore, I am executing this affidavit to attest to the truth of the foregoing
facts for whatever legal purpose it may serve;
