AFFIDAVIT OF ACKNOWLEDGMENT

I, TERESITO SENOLOS CARIDAD, of legal age, Filipino and


presently residing at Phase 5 A, Package 4, Block 16, Bagong Silang,
Caloocan City, do hereby depose and state:

1. That I am the biological father of BABYLYN TEVES TAOPO who


was born on May 22, 1991, at Quezon City, Metro Manila;

2. That BABYLYN TEVES TAOPO and BABYLENE TEVES TAOPO as


indicated in Certificate of Live Birth, Local Civil Registry
Number 91-23682 are one and the same person;

3. That the biological mother of my child is AMY TEVES TAOPO;

4. That I and AMY TEVES TAOPO have not entered into


marriage;

5. That I am executing this Affidavit to announce to the whole


world that BABYLYN TEVES TAOPO also known as BABYLENE
TEVES TAOPO is my biological child and by reason thereof, I
am allowing my child to use my surname.

IN WITNESS WHEREOF, I have hereunto affixed my signature


above my name this 9th day of September 2018 at Caloocan City,
Metro Manila.
