This refers to your contract PAMBAT-RRECIS-CIS-2018-BAT-ABU ABO -25M -27 ABU

ABO CIS located in Balanga, Bataan. Please be informed that as of June 15, 2019, you
have an actual accomplishment of 5.00% versus the planned accomplishment of
21.00% thus, you have incurred a negative slippage of -16.00% due to your fault caused
by limited equipment, materials, manpower which now falls under the “Terminal
Stage”.

Pursuant to Section III. A.2 of Annex I of R.A. 9184 and NIA Memo randum Circular No.
40, Series of 2017 – Administrative Action on Contract with Negative Slippages in
accordance with the Revised IRR of R.A. 9184, the implementing office shall initiate the
termination of contract.

In view hereof, you are hereby directed to “show cause” within seven (7) calendar days
from receipt hereof why your contract should not be terminated, pursuant to Section
IV.3 of Annex I of the Revised IRR of R.A. 9184.
