Instructions: Rate the faculty in each of the indicators below using the 5-point rating scale. 5 is
Outstanding, 4 is Very Satisfactory, 3 is Satisfactory, 2 is Fair, and 1 is Poor. If you wish to rate
your teacher 5, put a check mark opposite the indicator in the column of 5; if you wish to rate
him/ her 4, put a check mark opposite the indicator in the column 4, and so on.

1. Prepares quarterly report (March, June, September)
2. Prepares annual report (December)
3. Prepares terminal report of completed research
studies/ extension projects.
D. Human Relations (20%)
1. Maintains smooth interpersonal relationship with
fellow workers.
2. Maintains smooth interpersonal relationship with
the community.
3. Willing to assume responsibility and exudes
honesty in his / her work.
4. Maximizes the utilization of resources and explores
approaches at his / her initiatives and
innovativeness.
5. Establishes linkages.
