Assessing The Nails

This document is a performance evaluation checklist for assessing nails as part of a nursing student's clinical evaluation. It lists the purpose of nail assessment as determining nail and nail bed condition, presence of underlying diseases, and quality of nail care. The checklist then provides instructions on introducing oneself to the client, observing infection control, ensuring privacy, gathering medical history, and examining nail plate shape, texture, bed color, surrounding tissues, and capillary refill through blanch testing. Students are evaluated and results are documented in the client's record.
