MedleyTM Medication Safety System

Alaris Medical Systems


Introduction
 The ALARIS® Medley™ Medication Safety System is
designed to help you provide the best possible
patient care, by avoiding potential adverse drug events
associated with IV administration at the point of care.
This in turn helps you avoid the potential cost of
harm, both in human and financial terms. The
Medley™ Medication Safety System accomplishes this
by integrating the infusion, patient monitoring, and
clinical best practices at your facility.
System Components

 SpO2 module is
not currently
being used.
Programming Module

Pump Module

System On Button
Priming
 Close roller clamp
 Fill the drip chamber to 2/3 full
 To prime tubing and clear air from
SmartSite® Needle-Free valve y-sites,
slowly open the roller clamp
 Adult Patients: Invert and tap
SmartSite® Needle-Free Valve(s) while
priming
 Pediatric/NICU Patients: Once set is
primed, attach a sterile luer lock syringe
and aspirate air through the SmartSite®
Needle-Free Valve(s) into the syringe
 Close the slide clamp and roller clamp
Obtain pump from instructor to
practice the following tasks

Attaching Pump Module
 Press blue release button on pump module, tip at
45° angle
 Reattach the pump module, start at a 45° angle
 Maximum of 4 pump modules
Administration Set Priming & Loading
 Loading administration set
 Position upper fitment into
fitment recess
 Insert Flo-Stop® fitment into
recess below mechanism,
arrows point into Medley™
Pump Module
 Using fingertip, firmly push
tubing toward back of Air-in-

System Start-up
 Press “SYSTEM ON”  PROFILE: A name assigned
 NEW PATIENT? to a patient care area or
 “Yes” clears previous patient patient type.

 Select “YES”
 CRITICAL CARE
 Next – Screen displays previous
profile e.g. Critical Care?  MEDICAL SURGICAL
“YES” confirms same profile  OBSTETRICS
 Select “NO” to preview available  ONCOLOGY

 Select your patient’s profile on the  PEDS B (2.1-15kg)
LEFT of the screen  PEDS C (>15kg)
 Press Confirm  TRAINING
Programming a Basic Infusion
 Press CHANNEL SELECT
A
 Select BASIC INFUSION
 Press RATE, program 10mL/h
 Press VTBI (volume to be
infused), program 100mL
 Press START
 Lighthouse is GREEN to show infusion running
 Press PAUSE on pump module – lighthouse is now
YELLOW
 Press RESTART
 Lighthouse is RED when
infusion is disrupted
Changing Basic Infusion Rate
 Press CHANNEL SELECT A
 Press RATE, program 20 mL/h
 Press START

Note: It is not necessary to pause infusion to


change rate
Infusion Complete - KVO
 This is a SIMULATION
 Press Channel Select A
 Press Rate, program 50 ml/h
 Press VTBI, decrease VTBI to 0.1 ml by entering
.1
 Press Start

Note: KVO alert “Infusion Complete – KVO”, Red


and Green indicators illuminate lighthouse which
indicates that the KVO Rate is infusing.
Infusion Complete – KVO continued

 To return to primary infusion rate:


 Press Channel Select on Channel A
 Press VTBI, press Restore, note VTBI = 0.1 ml
 Change VTBI to 100 ml
 Press Start

Secondary Infusion
 Press Channel Select A
 Press Secondary
 Press Basic Sec
 Press Rate, program 200 ml/h
 Press VTBI, program 0.3 ml (to
simulate nearing end of secondary
infusion)
 Press Start
Remember to lower primary bag and unclamp secondary
Note: Plastic hanger that comes with secondary line should be
extended the full length when lowering primary bag to ensure full
infusion is given. When secondary is complete, the infusion will
automatically return to Primary Rate.
Policy
 Guardrail infusion: Used for the administration of
most continuous medication infusions and some IVPB
medications. If a medication is included in the
Guardrails library, the pump must be programmed as
a Guardrail infusion.
 Medications have minimum and maximum dosage
limits. If the dose is programmed above/below these
limits an alert will appear.

Mandatory use – Refer to Policy #465 IV Therapy Electronic
Infusion Device
 Press Channel Select B
 Press Options
 Press the Guardrails® Drugs
 Select Dopamine 400mg\250 ml
 Enter 95kg for patient weight
 Press Next
 Program VTBI as 250ml
 Program Dose as 5 mcg/kg/min
Note: If entering Rate, the dose will
 Press Start automatically be calculated. If
entering Dose, the Rate will be
automatically calculated.
Troubleshooting
 Air-in-Line – lighthouse is RED
 Message scrolls in channel message display
 Assess the amount of air in the line by pressing Restart
 Air passes the eye and resets the eye to detect more air
 If amount of air is clinically significant, remove air from the
line
 If no air is seen:
 Ensure the air sensor is clean and free from debris
 Ensure the set is properly seated in the Air-in-Line detector
Troubleshooting continued
 Occluded – Patient Side – Lighthouse is RED
 Partial Occlusion – Patient Side or Occluded - Patient
Side scrolls in channel message display
 Clear occulsion on patient side of the infusion below the
affected Pump Module and press Restart
 Flo-stop Open/Close Door – Lighthouse is RED
 Flow stop is in the open position (free-flow position)
with the door open
 Close roller clamp on the set or close the door then
press Restart to resume the infusion
Volume Infused
 Press Volume Infused
 Press Clear All
 Press Main Screen

Note: May clear each channel


individually by pressing soft
key corresponding to
applicable channel
Audio Adjust
 Press Audio Adjust
 Adjust audio using Softer
or Louder (1 through 5)
 Press Main Screen
Display Contrast
 Press Options
 Press Display Contrast
 Adjust contrast using Lighter or Darker
 Press Main Screen
Tamper Resist – Panel Lock
Prevents unwanted changes by unauthorized persons
 To lock panel
 Press and hold Tamper Resist Switch on back (3 seconds)
 Note: Main screen confirms “Panel Locked” when key is
pressed

Tamper Resist Switch

 To unlock panel
 Press and hold Tamper Resist Switch (3 seconds)
 Note: Main screen confirms “Panel Unlocked” and an
advisory tone will confirm tamper Resistant is off
Powering Off
 Individual Channel Off
 Go to individual pump module
 Press Channel Off for 1.5 sec (until beep is heard) and
release
Note: If this is the only pump module in use, this action will
Power Off the entire system
 Multiple Channels - System Off
 Press Options
 Press Power Down All Channels
 Power Down All Channels?
 Select No
