CHART A

A 32 year old female came to OPD with high fever for the past 12 hours with history of severe headache,
confusion, photophobia and neck stiffness. She has no significant past medical history and is not on any
medication.

CSF findings:

Normal

Naked eye appearance Cloudy

Opening pressure 300 mm H₂O in lateral decubitous position. 90-180 mm of water

Leucocyte count 3000 WBCs/ ml (>95% Neutrophils) 0-5 Wbcs/ml

Glucose level < 40% of serum glucose 60% of serum glucose

Protein level 3 g/L 15–45 mg/Dl

Questions: (1 mark each)

1. What is the most likely diagnosis? Give justifications.

2. Name any two organisms causing the above condition in elderly?

3. What is the cause of decrease in the CSF glucose level?

A 38-year-old female presents with 24 hours of headache, photophobia and mild neck stiffness, in
addition to stuffy running nose and congestion.She had perianal tenderness and rashes which on
examination revealed herpetic lesions.

CSF findings:

Normal

Naked eye appearance Clear

Opening pressure 300 mm H₂O in lateral decubitous position. 90-180 mm of water

Leucocyte count 150 cells /µL (Predominantly lymphocytes) 0-5 Wbcs/ml

Glucose level Normal 60% of serum glucose

Protein level 12 g/L 15–45 mg/Dl

examination multiple cervical and inguinal lymphadenopathy were found.

Leucocyte count 3000 WBCs/ ml (Predominantly 0-5 Wbcs/ml
