Reminders from Sir Alex:

- Budget Proposal for the EOSY must include the no. of candidates for
graduation, write in detail the requirements needed, gold seal
should be budgeted by box
- Be accurate in your budget proposal
- Medals (Provincial Government)
- Prepare our learners for Grade 3 ELLNA, Grade 6 NAT
- Please read D.O. 21, s. 2022 dated May 10, 2022 (Extending the
Effectivity of D.O. 8, s. 2021 to S.Y. 2021-2022)
- D.O. 25, s. 2022 (Remedial and Advancement Classes)

IMPORTANT: SFs Checking is focused on LIS Data

What to check:

 LIS generated SF-1 c/o Adviser


 LIS generated SF-5 c/o Adviser
 SF-10 (Form 137) c/o Adviser
 Diploma
 ECD Checklist
 Certificate of Completion for Kindergarten

For School Head:

 SF-4
 SF-6

School Checking Committee:

Chairman: School Head

Vice Chairman: School ICT Coordinator


Members:

Reminders for School Checking Committee and Class advisers:

(Focus on the accuracy and consistency of learner information,


instead of format and cosmetic aspects of SFs)

 The School Head is accountable and liable for any wrongful entry on
the forms therefore, the Division Checking Committee is not required
to put their names and signatures in SF-5
 Only the names of School Checking Committee shall be printed in the
SF-5 for signature
 School level checking is for all classes
 No. of school days is not included in the checking
 SF-2 will only be as reference in case of inconsistencies in SF-1 and
SF-5
 SF-9 can only be checked if SF-5 and SF-10 did not match
 PSA/NSO is preferred but not required or mandatory
 Brgy. Certification containing the basic information of the child or
Baptismal Certificate can be used as reference for learner’s identity
 LIS generated SFs are not subject for editing outside the LIS
 Any corrections should strictly be done in the system
 When printing, use black ink on A4 or long/folio white bond paper
 Advisers are not required to prepare additional local forms for the
ranking of honors
 Base the honors and other citations in the validated SF5
 Do not prepare and submit obsolete school forms
 NSO/PSA Live Birth Certificate should be attached to the SF-10 when
forwarded to High School
 Good moral certificate is no longer required for admission
 Learner must be enrolled in the LIS as soon as s/he attended classes
even one day attendance.

Checking/Validation of School Forms is spearheaded by the Planning and


Research Section with all the EPS.

Central office’s findings on school-based checking during pandemic:

 a lot of errors
 age entrance

of School Forms as prescribed in D.O. 11, s. 2018 (Guidelines and
Preparation of School Forms)

- need to go back to D.O. 11, s. 2018 in the checking SFs

Pandemic issues for not having division level checking:

 7 Divisions nationwide explained for having below 5 y.o. enrollees in


Kindergarten
 4 Schools in Northern Samar Division

Since N. Samar is now under Level 2, checking will be done through


combination of F2F and online.

Focus of Checking:

1. Kindergarten Age
 D.O. 20, s. 2018 (Cut-off Age for Kindergarten)
 D.O. 47, s. 2018 (Omnibus Policy on Kindergarten Education)
2. Proper Preparation of SF10 (Form137)
 D.O. 58, s. 2017 (Adoption of New School Forms for K, SHS, and
ALS)
 D.O. 4, s. 2014 (Adoption of the Modified SFs for Public ES and
HS)
 no insertion of school logo in SF10
3. Transfer of documents for T/O and T/I
 D.O. 54, s. 2016 (Guidelines on the Request and Transfer)
 Written or hard copy request is no longer required

4. Update Learners Status in LIS Tracking System

 D.O. 32, s 2021 (Guidelines on Enrollment for S.Y. 2021-2022)


 D.O. 37, s 2022 (Resumption of the Implementation of the Policy
on Checking of SFs as prescribed in D.O. 11, s. 2018

Kindergarten admission

Kindergarten Education shall mean one year preparatory


education for children at least five years old as a prerequisite for
Grade 1

 At least 5 y.o. as of 2019-2020 onward


 At least 5 y.o. as of August 31 if school opening is June
 At least 5 y.o. as of Sept. 30 if school opening is July
 At least 5 y.o. as of Oct. 31 if school opening is August

Mandatory Division Checking for all Kindergarten, Grade 1, Grade


6, Grade 10,11 and 12

Random Division Checking for other Grade Levels:

Very Small School – 100%


Small School – 50%
Medium School – 30%
Large School – 20%
Very Large School – 10%
