We always appreciate the opportunity your feedback gives us to improve our charity and our services. The more information we get on how we're doing, the easier it will be for us to be there for anyone with an illness they're likely to die from, and those close to them.
This form can also be used to report any potential instances of fraud. Marie Curie takes fraud very seriously – please give as much detail as you can to assist a thorough investigation. If the fraud is patient-related and in England or Wales, you can also report this to the NHS fraud hotline on 0800 028 4060.

We work hard to offer high standards of service at all times but sometimes things go wrong and there may be areas that we can improve on. We really value your feedback and want to look into any concerns you have. To do that, it is helpful for us to have as much information as possible about what happened and your areas of concern. Please include details of dates, times and locations and any staff members involved. If you have used one of our hospices or nursing services, please give the patient's name.
